All information contained in this application and all attachments hereto are true and complete to the best of my knowledge, and are made for the purpose of obtaining a lease/finance line of credit.
I (we) hereby authorize mbaleasing and any assignee, lender or funding service that may be utilized to obtain and use a consumer credit report on the undersigned, now and from time to time, as may be needed in the credit evaluation and review process and waives any right or claim they would otherwise have under the Fair Credit Reporting Act in the absence of this continuing consent. I (we) further authorize any bank, financial institution or trade reference to release credit information on my(our) accounts(s) to mbaleasing and/or its assigns.
I authorize mbaleasing to verify any of the information from whatever source it deems appropriate and I further authorize any of the above references to submit credit information to mbaleasing. I agree to notify you of any change in the condition of the company’s/applicant’s affairs, and this statement shall be construed by you to be a continuing statement of the conditions of the company’s/applicant’s affairs until written notice to the contrary is received by you.
It is understood that this application shall remain the property of mbaleasing, whether or not the lease/finance line of credit is granted, and that this constitutes an application only and shall not be binding upon either mbaleasing or the company/applicant mentioned above except for the certifications contained in this document. An electronic, photocopy or facsimile copy of this authorization with a copied, electronic or facsimiled signature shall be deemed to be binding, valid, genuine and authentic as an original-signature document for all purposes.

If you are "just shopping" at this time, do NOT give your Social Security Number or bank information to ANY leasing company. Too many inquiries on your credit report can hurt your chances for approval. IMPORTANT NOTICE
If your application for business credit is denied, you have the right to a written statement of the specific reasons for the denial. To obtain the statement, please contact the President of mbaleasing at the listed address and telephone, within 60 days from the date you are notified of our decision. We will send you a written statement of reasons for the denial within 30 days of receiving your request for the statement.
